Family Health Centers of San Diego is looking for a Social Services Manager. The Social Services Manager is responsible for providing administrative and operational management of assigned department programs and/or service lines. Supervises direct and indirect reports in support of social services integration and care management activities to improve the patient care continuum. Works collaboratively with department leadership and external partners to operationalize service lines and achieve strategic goals.

Responsibilities:

  • Build positive relationships, inspire others to perform at a higher level, prepare career development plans and retain an accountable workforce.
  • Coach and counsel staff and administer appropriate disciplinary action in a timely manner.
  • Participate in continuous improvement activities and ensure appropriate Lean tools (i.e., 5S, A3) are deployed and results are evaluated and/or measured.
  • Provide/ensure staff receive all new hire and annual trainings on time.
  • Oversee daily operations of assigned Social Services programs, grants, and/or service lines.
  • Support department development and continued expansion of episodic and longitudinal care management services.
  • Ensure quality of care management services, including health navigation, case management, transitions of care, and internal and external social resource navigation.
  • Foster and sustain FHCSD's community relationships including those with community hospitals, managed care plans, and other social services agencies.
  • Ensure timely and accurate reporting to internal and external stakeholders, includes program quality assurance and applicable regulatory compliance.
  • Develop and integrate social services staff, including case managers and social workers, as integral members of the interdisciplinary care team.
  • Work effectively with IT's Business Intelligence team to ensure strong performance metrics and measures of impact suggesting and implementing strategies based on outcome data.
  • Collaborate with clinic administration and ancillary leadership to improve upon the patient care continuum.
  • Prepare budgets, monitor expenditures, ensure revenue is adequate to support programs, and reviews purchasing requests related to assigned programs.
  • Serves as Site Manager for East County Wellness Center II and is responsible for all site-related duties.
  • Provides/ensures staff receive all new hire and annual trainings on time.
  • Perform other duties as assigned.

Requirements:

  • Bachelor's degree in Health Administration, Public Health, Social Sciences, Business Administration, Nursing, or closely related field required.
  • Master's degree in Health Administration, Public Health, Social Sciences, Business Administration, or relevant field preferred.
  • 3 years of progressive responsibility in healthcare or social services field required.
  • Bilingual in English/Spanish preferred.
  • Meets driving requirements.

In the spirit of pay transparency, we are excited to share the base range for this position, exclusive of fringe benefits or potential bonuses.

64,480.00 - 85,973.33

If you are hired at Family Health Centers of San Diego, your final base salary compensation will be determined based on factors such as geographic location, jurisdictional requirements, skills, education, and/or experience. In addition to these factors - we believe in the importance of pay equity and consider internal equity of our current team members as a part of any final offer. Please keep in mind that the range mentioned above is what we reasonably expect to pay for the role. Hiring at the maximum of the range would not be typical in order to allow for future and continued salary growth.
